The DS-156 is required for all applicants, and must be filled out completely online. After pressing "Continue", the applicant needs to print out and sign the PDF form.
Additional Forms Required for Some Applicants
All male applicants between the ages of 16 and 70 must complete the DS-157 form in addition to the DS-156.
Please note that many applicants experience delays on the day of their appointment because they have not filled out the DS-157 completely. Please use a continuation sheet for question 9 if needed. If this form is not properly completed, we will not be able to accept your application.
All students and exchange visitors (F-1, M-1, and J-1 visa applicants) must also complete the DS-158 Form
