Minor Passports

All children under age 16 receive 5-year validity minor passports.  Children under age 16 must appear in person and have both parents consent to be issued a US passport.  For information on children age 16 and 17, please see adult passport instructions.  To apply for a child's passport, please make an appointment on our online appointment system and bring in the following on the scheduled day: 

  1. 1. Passport Application Form DS-11.  Completed but not signed.  (List local address for Mailing Address although passport will not be mailed.)

    to complete online and then print. You will sign the form in front of the consular officer.  
  2. One photograph (Photo must be 2 inches by 2 inches SQUARE, recent, and with white background as outlined here (PDF 45KB).  If you have an incorrect photos, you will have to make a second trip to the embassy to complete your child's application.)
  3. The child's current passport and 1 photocopy of the biographic page(s) and any amendments.
  4. Child's original birth certificate showing both parent's information plus 1 photocopy.  If you do not bring your child's birth certificate, we will be unable to accept the child's passport application.
  5. Parents' consent - Both parents must come in person with valid government issued ID, plus 1 photocopy of each ID, to sign for the child's passport.  (If one parent is out of the country, please provide an original, notarized Statement of Consent (PDF 157 KB) from that parent.  If one parent has sole custody, that parent must show a legally binding court document granting sole custody. If one parent is deceased, please provide an original certificate of death.)
  6. The fee for a passport for a minor is US$105 or the equivalent in UAE dirhams.  This is payable in U.S. dollars, dirhams, or by credit card.

If the copies requested above are not provided with the application, applicants will be charged a photocopying fee of US $1 per page. 

The new e-passport is printed in the U.S. and requires about two and half weeks to process.  You may keep your child's current passport and have it cancelled when you collect his/her new passport from the Embassy (we will not cancel the UAE residence visa).
